Tracking your symptoms day to day can help you spot patterns, understand your unique experience, and feel more in control of what’s happening. It’s also a great tool to bring to your doctor or healthcare provider, so you don’t have to rely on memory during appointments.

Use the checklist to track symptoms using the numbers from the checklist, your menstrual cycle and jot down notes of any potential triggers you notice, like certain foods, alcohol, stress, or busy days. You can either print it out or type your notes directly on the page. There’s no “perfect” way to do this. The goal is simply to gather insight so you can make choices that support you. This is about curiosity, not judgment.
